Integrated Community Supports (ICS) Program

At TotalCare Services LLC, we offer an Integrated Community Supports (ICS) program designed for adults with disabilities. This program focuses on empowering individuals to live independently within their own homes and communities while receiving the necessary support to achieve their personal goals and maintain independence.

Apartment-Based Support

The ICS program is delivered in an apartment setting, where each client has their own private unit, including a kitchen, bathroom, and bedroom. Support services are available around the clock, up to 24 hours a day, depending on the client’s needs. If required, additional services such as nursing or home care can be coordinated to ensure comprehensive care and support.

Services Offered

We offer a comprehensive range of services aimed at promoting independence, supporting personal goals, and maintaining secure housing. Our services also work to reduce the need for crisis intervention. Below are some of the key services provided:

  • Community Participation: Our staff assists clients in safely navigating the community, accessing local resources, and meeting transportation needs. We support leisure activities, recreation, and socialization, helping clients engage with their surroundings and enjoy their favorite pastimes.
  • Health, Safety, and Wellness: We prioritize the well-being of our clients by supporting healthcare appointments, social services, and daily self-care activities. Our staff assists with Activities of Daily Living (ADLs) and helps clients achieve their health, safety, and wellness goals.
  • Household Management: Maintaining a well-managed household is crucial to independent living. Our staff offers guidance, training, and supervision in household care, safety, financial budgeting, meal planning, and more. We empower clients to manage their homes efficiently while maintaining a secure living environment.
  • Adaptive Skills Development: Our team helps clients develop the skills necessary to thrive in their daily lives. From crisis prevention strategies to positive behavior reinforcement and problem-solving techniques, we are committed to helping our clients enhance their self-sufficiency and overall well-being.

Eligibility Requirements

To participate in our ICS program, individuals must meet the following criteria:
  • Age: 18 years or older.
  • Disability or Mental Health Conditions: We serve individuals with mental illness, brain injury, or other disabilities. Our services are designed to address and accommodate these specific needs.
  • Active Participation: Clients are encouraged to actively participate in setting and achieving their personal goals, fostering a collaborative and empowering environment.

Application Process

We offer support throughout the application process to ensure a smooth transition into our ICS program. Public funding may be available through waiver programs, such as the Community Alternatives for Disabled Individuals (CADI) waiver or the Brain Injury (BI) waiver, CAC, and DD. Private payment options are also available for those who do not qualify for public funding.

Our application process includes:

  1. Submitting a Referral/Housing Application with supporting documents.
  2. Attending a Meet & Greet Tour after passing the initial screening.
  3. Completing a financial review and an intake assessment, including a physical exam and required documentation.
  4. Finalizing the assessment and submitting personal identification documents (birth certificate, MN ID, etc.).
  5. Scheduling a move-in date and completing any final needs, such as transitional services, rent/deposit payments, and missing documentation.
  6. Signing the lease and moving into your new home.
